What is the Caregiver Wellness Survey? The Caregiver Wellness Survey is a national study of frontline direct caregivers, including nursing assistants, home health aides, personal home care assistants, and family caregivers who assist elderly and disabled clients or loved ones with activities of daily living. What is the purpose of the Caregiver Wellness Survey? The purpose of the survey is to hear—directly from you—how social networks, stress, turnover, and job/role satisfaction impact the health and well-being of frontline direct caregivers. Your participation will help to generate coping strategies to support frontline direct caregivers providing care for the elderly and disabled. What do I have to do if I agree to be a part of the Caregiver Wellness Survey? Fill out the questionnaire online. The questionnaire will include questions about: - Training

After completing the survey online, a PDF file of Caregiving in the New Millennium: A Survival Guide for Caregivers, a $15 retail value, will be included on the thank you page at the end of the survey. How confidential is the information I give you? The information you give is kept private. No information will be given to anyone. Your answers will be grouped with answers given by other frontline direct caregivers who care for the elderly and disabled in the United States.
